Governor Ducey signed an Executive Order 04.22.2020. This order is for Providers to demonstrate, in a request, a need for an exemption from Executive Order 2020-10. Link for the order.

Public health can assist with support and coordination of COVID-19 testing and shipping logistics with the your facility and Arizona State Public Health Lab (ASPHL). If you need swabs for testing, you can submit a request to ADHS via the “request-for-materials” pdf. The kits can be put under miscellaneous in the form. Please be specific about what you need (e.g. swab and viral COVID media). For PPE request visit the MCDPH site and submit your request –https://www.maricopa.gov/5523/PPE-Supply-Requests.
